Out of the many failure modes I've personally explored one of the most enduring is what I call the "hell yeah."
It's when you're contemplating a play and can't quite totally wrap your head around it and you say "hell yeah that'll work" and you just dive in.
This often comes in the wake of several other "hell yeah" plays that panned out, giving you a feeling of invulnerability.
But then as you stare at the board with only that one remaining freecell and you come to grips with the ultimate emptiness of being...
Now, that said, I just wiggled outta my last sticky situation so I'm empowered now to really swing for the fence. Watch for a crash and burn very shortly.

I often have "what the hell" moments, where I see no way to win a particular game. So I do something brilliant like just start clicking cards with no rhyme or reason. Amazingly sometimes it works out, but not that often.
We are very glad/tickled to finally have the secret of your 491 streak in 8x2s revealed. It's hard to keep such powerful information hidden forever. Still, very impressive nonetheless. 🏆
